Signing Electronic SIE forms through Adobe Sign

The purpose of this document is to provide details on how the numbered signature prompts appear on the form.

Forms

Overtime Eligible - hourly employees (will require IHR-Compensation signature regardless of amount)

Overtime Exempt - salaried employees (will only require IHR-Compensation signature for amounts over $10,000)

3-Requesting Unit

4-Second Requesting Unit signature (optional)

5-Requesting College

6-Second Requesting College signature (optional)

7-Employee's Home Unit

8-Second Employee's Home Unit signature (optional)

9-Employee's Home College

10-Second Employee's Home College signature (optional)

11- Not an available option on the screen as it automatically sends to IHR-Compensation (ihr-compensation@illinois.edu)
