AITS - Service Desk - Conflict Form Information and Support (COCI & RNUA)
Use this article to find the correct conflict disclosure form, reporting guidance, and support contacts. The form you use depends on your employee classification and University appointment.

Civil Service Employees
Complete the annual Civil Service Conflict of Interest Report Form, including when you have no conflicts to report. Part-time employees must also report.
Academic Professionals at UIUC, UIS, and System Offices
Use START myDisclosures to complete the Report of Non-University Activities (RNUA).
Academic Professionals at UIC
Follow the UIC Research COI Disclosure Profile process. Covered individuals must complete their annual profile even when they have no outside activities to disclose.
System Offices Employees with UIC Connections
- Employees located at UIC without a UIC appointment continue using START myDisclosures.
- Employees with any UIC appointment must complete both an RNUA in START myDisclosures and a UIC Research COI Disclosure Profile.
- System Offices investigators on UIC sponsored programs must also complete a UIC Research COI Disclosure Profile when prompted.
Complete Your Disclosure
Civil Service Employees
- Open the Civil Service Conflict of Interest Report Form.
- Sign in and follow the form’s instructions.
- Disclose outside income-producing activities, relationships with entities doing business with the University, and relevant family conflicts.
- Submit your report. Reports with disclosed conflicts automatically route to your supervisor for approval.
Contact your supervisor, unit head, or staff HR office if you need help interpreting a question.
Academic Professionals using START myDisclosures
- Open START myDisclosures and sign in with your NetID and password.
- Select +Start new RNUA Disclosure.
- Complete retrospective time reporting if prompted.
- Answer the screening questions and enter or update applicable activities.
- Review and certify your answers, then complete submission.
Employees with nothing to disclose must still complete certification and submission.

Academic Professionals using UIC Research COI
Sign in to UIC Research COI and follow the Disclosure Profile instructions. Job aids and training are available in the application’s Help Center.

For sign-in or loading problems:
- Use account recovery for password or authentication issues.
- Check the University system status page for reported outages.
- Try clearing your browser’s cache and cookies.
When requesting help, provide the form name or URL, employee classification, campus or System Offices affiliation, exact error message, and steps already attempted. Remove sensitive disclosure details from screenshots. Do not send passwords or authentication codes.

Incorrect Employee Information
If START myDisclosures displays incorrect appointment or personal information, contact your HR department to correct the information in Banner.
Deadlines and Resources
Follow the deadline in your reporting notice and the applicable reporting schedule. UIC uses a separate schedule.
Academic employees may need to submit additional disclosures and obtain prior approval when outside activities change during the year.
