ATLAS FormBuilder - Question Types Explained
Questions in FormBuilder come in many types, each with distinct functionality and use cases. These question types often possess their own unique settings as well, which allow for finer customization of their behavior.
To modify a question's type, first create a new question or begin editing an existing question. Instructions on how to accomplish this can be found at [Link for document 151589 is unavailable at this time].
Once you've opened the editing panel for your question, locate the field labeled Type.
As the name implies, this field's drop-down menu allows for selection from a wide range of question types. Depending on which type is selected, additional settings may appear below.
FormBuilder's available question types are as follows:
Single Line Text - Allows for entry of text in a single line. Good for questions that require a short-form written response.
- Associated settings:
- Default Value: A text value that is entered into the question's field by default in a new form response. This setting accepts text merge.
- This text must match a pattern: When selected, the question will require its value to match a pre-specified format. The formats available for selection are...
- Telephone Number
- US Zip Code
- Email Address
- Web URL
- This text must match a regular expression: When enabled, causes the appearance of an additional setting that asks for the entry of a regular expression (regex). The question will then require its value to match with the value(s) specified by that regular expression.
- Default Value: A text value that is entered into the question's field by default in a new form response. This setting accepts text merge.
Multi-line Text - Allows for entry of text in a multi-line text box. Good for questions that require a longer written response or information that cannot fit on a single line.
- Associated settings:
- Add max word count: When selected, causes the appearance of an additional setting that accepts a numerical value to represent the question's "max word count." If a user's answer possesses a word count higher than this value, the answer will be rejected.
- Default Value: An optional text value that is entered into the question's field by default in a new form response. This setting accepts text merge.
- This text must match a regular expression: When enabled, causes the appearance of an additional setting that asks for the entry of a regular expression (regex). The question will then require its value to match with the value(s) specified by that regular expression.
- Add max word count: When selected, causes the appearance of an additional setting that accepts a numerical value to represent the question's "max word count." If a user's answer possesses a word count higher than this value, the answer will be rejected.
Date / Time - Allows for the selection of a specific date and/or time using a date-picker widget.

- Default Value: An optional value that is entered into the question's field by default in a new form response.
- Minimum: Specifies the earliest chronological date and/or time that the question will accept.
- Maximum: Specifies the latest chronological date and/or time that the question will accept.
- Date Type: Specifies what type of temporal information the question accepts. The available options are...
- Single Line Text: Allows form responders to enter a single line of text, which is useful for short (single word and single sentence) responses.
- Multi-line Text: Allows form responder to enter a larger amount of text. For example, this can be used for short answer responses.
- Date / Time: The form responder selects a date, a time, or both from the designated timeframe you define.
- Number: The form responder can enter a numerical value within the designated range you define. This number can include decimals, or you can set it to only accept whole numbers.
- Campus Person (NetID or UIN): This question type collects the form submitter's assigned NetID or UIN. This question can be used to populate other questions - (first name, last name, email, expected graduation, )
- Single Checkbox (True/False): The form responder can check a box if the question value is true (unchecked for false). This option is useful for verifying information.

- Check Boxes: The form responder can select multiple options out of the list you provide. For more information, check out FormBuilder - How to Create Checkbox Questions.
- Select List: The form responder can select multiple options out of the list you provide. For more information, check out FormBuilder - How to Create Select List Questions.
- File Upload: The form responder can upload a file to the form. This could be useful in a situation where responders need to include a resume or other documents on an application.
- Time Stamp: States the date/time when a form is submitted. Cannot be manipulated by the form responder or edited by the administrator because it is automatic.
- User Stamp: States the name and email of the form responder that last made changes to the form. Cannot be manipulated by the form responder or edited by the administrator because it is automatic.
- Signature: Useful for electronic signatures.
- Multiple Questions: Allows you to create a subgroup of questions within a question. This is useful when simplifying your form (assigning and sequencing questions).
- Guest: In certain forms like event registrations, the form responder may want to include their guest(s) in their registration. This option allows form responders to include the number of guests.
- Person Attribute: Names specific attributes about the form responder.
